Rapport og anbefaling vedrørende ulemper og fordele ved henholdsvis at undlade eller gennemføre lumbalpunktur for patienter i blodfortyndende behandling mistænkt for akut bakteriel meningitis.
Denne vejledning er tiltænkt diagnostik og behandling af formodet Lyme neuroborreliose hos voksne. Vejledningen er udarbejdet som et supplement til Borrelia klaringsrapport (2014).

Der er lige nu massivt fokus på læger der har begået fejl. Aktuelt drejer det sig om behandling af meningokoksygdom. Bestyrelsen for Dansk Selskab for Infektionsmedicin (DSI) kender ikke til detaljerne i de konkrete sager og har ikke set sagsakterne. Dette er derfor en generel udtalelse som tager udgangspunkt i udmeldingen fra Patienterstatningen vedr. de tre meningokok-sager.

isikoen for bakteriel infektion og sepsis er svært forhøjet ved manglende miltfunktion som følge af splenektomi samt kongenit eller funktionel aspleni. Alvorlige infektioner skyldes primært Streptococcus pneumoniae, Haemophilus influenzae og Neisseria meningitidis.
Risikoen for svær infektion ved malaria og dyrebid er også markant forhøjet.
Vaccination, antibiotika profylakse/tidlig behandling og adækvat patient information er afgørende for forebyggelse af infektioner.

'Linezolid manifests a rapid and dramatic therapeutic effect for patients with life-threatening tuberculous meningitis' af Feng Sun og kolleger publiceret i Antimicrobial Agents and Chemotherapy - et retrospektivt studie fra Kina, der antyder at Linezolid kan give et godt bidrag til behandling af tuberkuløs meningitis.
Johansen kommenterer: Et trist resultat: I 2013 rapporterede Ruslami et al. (Lancet Infect Dis. 2013 Jan;13(1):27-35) nedsat mortalitet ved IV administrering af en øget dosis Rifampicin og Moxifloxacin i blot 14 dage til ca. 60 patienter med tuberkuløs meningitis - efterfulgt af standard behandling. Der var derfor store forventninger til denne store kliniske trial, hvor effekten af øget Rifampicin dosis samt et flourquinolon (per oralt) blev undersøgt på ca. 700 Vietnamesiske patienter.

A previously healthy 52-year-old man presented to the emergency department with acute onset of pain and swelling of both knees, ankles, wrists, and shoulders. He was febrile with a temperature of 38.5°C and was immobilized due to the severe pain. Physical examination showed marked effusion in both knee joints, both ankles as well as the left wrist. There were no clinical, laboratory or radiological signs of any respiratory, abdominal, urinary, or neurological infection, nor cutaneous lesions.

Neisseria meningitidis (the meningococcus) is a diderm aerobic diplococcus, with a ‘coffee-bean’ shape, associated with life-threatening meningitis and septicemia, as well as pneumonia, septic arthritis, pericarditis or, more recently, urethral infections. N. meningitidis is a commensal organism in the nasopharynx and the throat of humans and is unable to survive outside of the human body.
