Could this be encephalitis? When I started working at an infectious diseases department, this was one of my most feared scenarios. The number and nature of the potential pathogens involved was overwhelming and I felt incompetent and unsure. Encephalitis was difficult to understand, diagnose, and manage. Even the very definition was vague, and it seemed like every acutely hospitalized patient with altered mental status could be suspected of encephalitis.
