We recently read a study published by Kildegaard et al.[1], which evaluated the safety and efficacy of Tdap vaccination during pregnancy based on the Danish nationwide registry data system. Despite the rigorous design of the study, there are still several limitations. Firstly, the study period (2019-2023) spanned the outbreak and prevention period of the COVID-19 pandemic. During the pandemic, people's awareness of hygiene and protection increased, and social distancing measures reduced exposure risks.

Purpose of review . Cases of whooping cough (pertussis) have rebounded strongly from the very low incidence observed during the pandemic. This re-emergence is characterized by changes in epidemiology. Here we describe the importance of genomics to monitor and understand the drivers to these changes. Recent findings . Changes in the genotype of strains isolated during recent outbreaks suggests that the pandemic disturbed the global Bordetella pertussis population structure.

Pertussis remains a significant global health challenge despite widespread immunization programs [1]. The introduction of whole-cell pertussis vaccines (WCVs) in the 1940s and subsequent transition to acellular pertussis vaccines (ACVs) initially achieved substantial disease control [2]. However, recent epidemiological data have revealed concerning shifts in pertussis dynamics, with increasing cases reported even in populations maintaining high vaccination coverage [3,4].

In 2024, Slovenia recorded the highest number of confirmed pertussis cases since the introduction of pertussis vaccination in the national vaccination program. In November 2023, the number of reported pertussis cases began to rise in Slovenia, peaking in March 2024. A total of 1,916 confirmed cases were reported in 2024. 52% of cases occurred in individuals under 15 years of age, with 5% of these being infants under 1 year of age.

Vaccinating women with Tdap-IPV in pregnancy was safe and well tolerated in a sub-Saharan African setting and boosted the quantity and quality of pertussis-specific antibodies in infants in early life. Although Tdap-IPV was associated with relative blunting of the immune response to the DTwP primary vaccination series, pertussis-specific antibody quality and memory B-cell responses were nevertheless preserved, regardless of the vaccine given during pregnancy.
